PROVIDER ENROLLMENT

Based on the identified needs of the community, AC LME is currently seeking applications for providers interested in obtaining a Memorandum of Agreement (MOA) for the following services:

  • Outpatient Treatment (Independent Practitioners)
  • Psychiatric Services
  • Substance Abuse Services for Adolescents and Adults
  • Services targeting the Hispanic/Latino population.  Note: Proposed programming, policies, and procedures shall address more than the availability of bilingual staff and shall demonstrate a solid understanding of the complex cross-cultural issues involved in the treatment of youth and families served. 

AC LME will honor requests for MOA’s with provider agencies that are currently endorsed by another LME to provide enhanced benefits.  We are not accepting applications for new services requiring endorsement (i.e. Child/Adolescent Residential Services-Group Type, Intensive In-Home Services, Community Support Team, etc.).

Any provider interesting in obtaining an MOA with AC LME must submit a completed Application for Enrollment with all required supporting documentation to:

The AC LME is no longer accepting requests for Letters of Support for .5600A or .5600C Supervised Living facilities. You may request a Letter of Support for other facilities by contacting Carmen Morrow, Training Coordinator at (336)513-4200, ext. 4108 or via email at cmorrow@acmhddsa.org.
